The Center Moriches Fire Department is proud to be a sponsor of the “File of Life” program. Our Fire Department promotes this important life saving program by encouraging all our residents with medical conditions and special needs to get their File of Life packet today.
What is the File of Life ?
The File of Life is a red plastic envelope with a magnetic strip on the back, which has an insert card within that list personal medical data. The completed File of Life packet is placed on the refrigerator door in the kitchen. It should be in plain view for emergency responders who are trained to locate the File of Life packet when responding to your home.
How much does it cost ?
The "File of Life" packet is provided free of charge to all residents of Center Moriches Fire District.
What do I do with my File of Life Packet ?
When you receive your packet be sure to fill out all the information as thoroughly as possible. Use a pencil not a pen, this will enable you to change any information regarding your medical history if needed. Place the File of Life sticker on the front door of your home to alert emergency responders of your File of Life packet. Keep a copy of your health care proxy or DNR with your File of Life packet so emergency responders can respect your wishes when it comes to your medical care.
